Immunization Schedule

  • 2 Month - Pediarix (DTaP, Hep B, IPV), HIB, Prenvar 13, Rotarix
  • 4 Month - Pediarix (DTaP, Hep B, IPV), HIB, Prevnar 13, Rotarix
  • 6 Month - Pediarix (DTaP, Hep B, IPV), HIB, Prevnar 13
  • 1 Year - Varivax (chickenpox), MMR, Hep A#1
  • 15 Month - DTaP, Prevnar 13, HIB
  • 18 Month - Hep A#2 if at least 6 months since Hep A#1
  • 2 Year - Hep A#2 if not received at 18 months
  • 4 Year - Kinrix (DTaP/IPV), MMR, Varivax (chickenpox), Hep A if not already received
  • 9 Year - Gardisil discussed (HPV two or three dose series depending on age first received)
  • 11 Year - Meningococcal ACWY (Menveo or Menactra), TDaP, Gardisil if not already received
  • 16 year - Meningococcal B, Booster Meningococcal ACWY (Menveo or Menactra)
