Access Your Patient Portal Account
You can access your child’s chart as well as some labs, growth charts, immunization records and more. Please call the office for a PIN that can be sent via text or email to register your account. If you are a new patient, you will receive a link via email to begin portal registration once an appointment is scheduled, please follow the steps in the QRG below.
Our patient portal is a secure and convenient way to access information 24/7. The portal is designed to make your life easier. Some of the features currently available include:
- View and print immunization records
- Send non-urgent messages to the office
- See upcoming scheduled appointments and access previous visit summaries.
- Send update information regarding contacts, address, and other demographic information
- Request and print completed school, camp and sports physical forms (yearly well visit is required for this)
- Pay online
- View, and print billing statements

If you had an account with our previous patient portal (more than 3 years ago) you will still need to create an account with the new portal. The previous portal is no longer available.
